Starting from November 16, 2020, foreign passengers taking flights to China departing from Slovakia are required to apply for a "Health Declaration Certificate" online. Foreign passengers are requested to log on to the website http://hrhk.cs.mfa.gov.cn/H5/, filling in the information online, declaring their health status and uploading the Negative Nucleic Acid and Anti-Body Blood test certificates and other required documents. The Chinese Embassy in Slovakia will review those and issue the "Health Declaration Certificate" (electronic version) online via the website. Result will be displayed in the form of a QR code with "HDC" logo (see Sample below). Upon boarding, foreign passengers are required to present the electronic version or a printed version of the QR code to the airline company for verification.
